My Life My Way – a CLEAR© model for Dementia Care

Overview

This pilot project proposes to integrate two services: Age NI’s My Life My Way© and the Northern Health and Social Care Trust’s (NHSCT) CLEAR© Dementia Care to deliver a unique collaboration between the voluntary and health and social care sectors. This will be achieved by extending the CLEAR© Dementia Care training model to: 50 carers of people living with dementia (PLWD) and 5 Age NI’s My Life My Way volunteers.

Project Details

The aim of the project is to support people living with dementia to remain resident at home for longer by identifying and supporting unmet need, as well as provide vital support to carers through volunteers. This service will be based in the Antrim / Ballymena area of the NNHSCT in Northern Ireland. Following assessment of the PLWD, carers will be provided with advice and training on the non-pharmacological management of behaviors by NHSCT Dementia Home Support Team (DHST) and a plan for addressing their unmet needs will be put in place. An Age NI’s My Life My Way volunteer, also trained by the DHST, will be matched with the carer to help implement the plan as well as widen the community support network for the carer and the PLWD. Based on evidence of the successful outcomes of both My Life My Way and CLEAR© programmes to date  expected outcomes for this project will include: reduced care-giver burden; widened social inclusion and a collaborative model of health and voluntary sector support which can be scaled across Northern Ireland.
